1. Questionnaire

(Mandatory for all clients)

Please print and complete this Client Drop Off Questionnaire form:

  • All clients, whether new or repeat, must complete this step

  • Returning clients will receive a copy in the mail (mailed on February 1, 2026)

  • Blank copies are also available in the literature holder located at our office at 2289 Fairview St, Unit 107

  • We do NOT require the Questionnaire form to be dropped to the office in advance of all other documentation; please assemble all documentation (as listed below) and drop together with the Questionnaire form in the same envelope


2. Authorization

(Mandatory for all new clients)

If you have already signed an Authorization form for Tax-Pertise Services in the past and have not cancelled it, proceed to Step 3

  • This step is NOT required if you have already signed an Authorization form for Tax-Pertise Services in the past

  • This step is mandatory for all new clients, as it allows us to access your account with Canada Revenue Agency to ensure accuracy and completeness when preparing your return

  • On your Drop Off Questionnaire form (Step 1 above), you will need to confirm with us if you have access to your ‘My Account’ online with CRA at https://www.canada.ca/en/revenue-agency/services/e-services/cra-login-services.html

    If you have access to your CRA account:

    • Once our office has received your package, we will submit the request for authorization and be in touch with you to confirm once it is available for your approval online (please do NOT initiate the request yourself)

    • For detailed instructions about how to approve the request, please visit the Authorization Instructions page on our website

    • Note that this will expire 10 business days from the date we submit the request, so we ask that you log in to complete the approval process ASAP once we have advised you of submission

    • Your file will not be added to our queue until this process is complete

    If you do NOT have access to your CRA account:

    • In the package that you drop to our office, you will need to include your most recent Notice of Assessment (issued at least 6 months ago), along with the Questionnaire form (Step 1 above) and all relevant documents per the ‘What to Include’ listing below

    • We will require information from this assessment to produce the Authorization Request form, which you will need to sign before we can proceed

    • Your file will not be added to our queue until this process is complete

  • Once authorization is established via either method described above, a CRA representative may call you within a day or two to confirm that you have signed an Authorization Request form or, alternatively, you may receive a notice in your ‘My Account’ with Canada Revenue Agency to confirm that the authorization has been established


3. Delivery

Seal all documentation (see list below), including the Client Drop Off Questionnaire in an envelope and deliver to our office at 2289 Fairview St, Unit 107.

  • No appointment is necessary to drop off your documentation

  • If you arrive when the office is open, please use the drop box located just inside our office door

  • If you arrive when the office is closed, you may still deliver your package via the mail slot in our office door (do not leave your documentation in our mailbox, as this is not a secure location)

We will be in touch as we are working through your return if there are any questions or when it is complete. Please ensure to include the best phone number to reach you at the bottom of your Client Drop Off Questionnaire form

Note that we are not accepting intake via email at this time


What to Include:

  • All income information (T-slips such as T3, T4, T5, etc. as well as any gain/loss reports, business income/expense worksheets, rental income/expense worksheets, etc., as applicable)

  • Documentation and receipts for all deductions, expenses, and tax credits that you wish to claim (excluding medical expenses - see below)

  • Notice of all Income Tax Installment payments made during the year (you should receive a notice in February with a confirmation of all payments received by CRA)

  • Details of Property Taxes and/or Rent paid in Ontario (applicable to low income households and seniors 65 years of age and older)

  • A copy of all pages of your most recent CRA Notice of (Re)Assessment (in most cases, this will be for tax year 2024)

    • Not mandatory if Tax-Pertise Services has representative access to your CRA account already established

    • As noted in Step 2 above, this is required for any new client who does NOT have access to their CRA account online, so that we may begin the process to establish authorization for representative access to their CRA account

Please note re: medical expenses:

  • Please retain all medical receipts at home (other than those noted as exceptions below) and provide us with the total out of pocket expenses paid for each family member in the appropriate section on page 2 of the Client Drop Off Questionnaire

  • Exceptions: Please include the following receipts with your package and note if they are already included in your total provided:

    • Accessibility expenses

    • Attendant care expenses

    • Annual receipts from retirement or nursing homes

  • Please ensure you check the dates on your receipts and include only those eligible for the 2025 tax year
